When it comes to improving the health and well-being of our communities in County Durham, a joined-up approach is vital. For the last few years the council and the local NHS have been working hard to adapt the health and social care systems to meet the changing demands of our communities; we have developed new ways of working based on innovative partnerships, providing more care in people’s homes and breaking down barriers between services.
We have come a long way, but we are also ambitious and determined to do more to improve the outcomes for our local communities. We are currently seeking to appoint to a joint post which will be key to the future success of our community health services. The Director of Integrated Community Services will be responsible for a wide remit including community nurses and social work teams, so that we better support primary care as well as individuals. Ultimately, the new postholder will drive effective integration between health and social care through collaboration with stakeholders and partners, with the aim of reducing avoidable hospital attendances and admissions, improving reablement and rehabilitation, streamlining and co-ordinating services, increasing productivity, and enhancing choice and personalisation.
Candidates could come from local government or the health sector and will need experience of leading large and complex teams, developing integrated services, and improving outcomes for communities. You will need to be a skilled relationship manager, with a ﬂ exible approach and a determination to better support our 500,000 residents.
